After the visit
The decision becomes documents the patient keeps: the take-home, the plan, the letter, shared under your name.
Most of what is said in the room is gone by the time the patient reaches the car. Mosaic turns the visit into documents: what was discussed, what was decided, what comes next, in the patient's language, drafted from the note and the answers, signed by you before anything is shared.

Documents
What Mosaic draftsEvery document a visit produces, drafted from the note and the chart, in the structure your practice uses, for your signature.Patient handouts and take-homesFrom an answer or a note, Mosaic drafts what the patient takes home: what was discussed, what to do, why, in plain language, with your instructions and the sources attached.Plans, letters and referralsThe plan for the patient, the letter to the colleague and the referral are drafted from the note and the chart, ready for your signature.Share with the patientOne link, sent from the practice. The patient opens the documents on the phone or on the computer, keeps them, and can ask about them.Where documents liveOne document, three lists: with the visit it came from, with the patient, and in Documents. Its status says whether it is a draft, signed, or shared. The patient's side
The patient appEverything from the visit, kept: the summary, the plan, the results explained, the medications, and an assistant that answers questions about what the physician shared. What the patient can enterAnswers to the anamnesis, home measurements, questions before the next visit: what the patient enters lands in the chart, dated, and in the summary before the visit. The guided chatThe patient app answers inside what you shared and stops where a decision begins. That boundary is the product, not a limitation of it.Questions that reach youWhat arrives is never a chat history. It is a sheet with the question, the plan it touches, the context from the chart, what the patient already heard, and a reply you approve.
