What the patient can enter
Answers to the anamnesis, home measurements, questions before the next visit: what the patient enters lands in the chart, dated, and in the summary before the visit.
What can be entered
- Anamnesis answers
- The questionnaire the practice sends before a visit, answered on the phone.
- Home measurements
- Blood pressure, weight, glucose, symptoms, entered by hand or from a device, in the plan the physician set.
- Questions and messages
- A question for the next visit, or a message to the practice with a deadline.
- Documents
- A letter from another physician, a photo of a report, uploaded for the practice to see.
The blood pressure entry in the patient app, morning and evening values of one week as a small chart; next to it the same values in the physician's summary under 'Entered by the patient'.
Where it lands
Everything the patient enters is a chart entry marked as entered by the patient, with the date. The summary before the next visit shows it against the chart: a blood pressure above the target the physician set, a new medication the patient mentions, a symptom she reports.
What the practice decides
The physician decides what a patient can enter and when: a blood pressure plan for four weeks, a questionnaire before the recall, nothing at all. Entries that need an answer go to the practice with a deadline the patient can see.
