Plans, letters and referrals
The plan for the patient, the letter to the colleague and the referral are drafted from the note and the chart, ready for your signature.
The patient plan
Tasks, appointments, medications and the next visit, drafted from the decision, dated where the decision names a date. The patient sees it in the app as her plan; the tasks can be ticked off; appointments carry reminders.
A patient plan as the physician sees it before signing: three medications with dose and time, four tasks with dates, two appointments, and the note 'Draft, not yet shared'.
Letters and referrals
A referral letter or a letter to a colleague draws on the whole chart: the reason, the relevant history, the findings with their dates, the question you want answered. It is written in the structure the receiving colleague expects and cites the documents it summarizes, so that the colleague can check it.

Every document Mosaic writes is a draft with your name on it only after you have signed it. Signing puts the document into the chart, dated, and makes it shareable.
