Ambient documentation

With the patient's consent, Mosaic listens to the visit and writes the note in the structure your practice uses. You read, edit and sign.

Before you start

The patient agrees to the recording, once per visit. The practice keeps a record of the consent. Recording starts with one tap and shows that it is running the whole time.

During the visit

You talk with the patient. Mosaic listens, tells the voices apart and keeps the conversation with the chart it already knows, so that a value the patient mentions is matched to the value in the chart. You can ask a question in the middle of the visit; the answer appears on your screen and does not enter the note unless you say so.

The note

When the visit ends, the note is ready in your structure: anamnesis, findings, assessment, plan, or the template your practice uses. Every sentence in the note can be traced to the passage in the conversation it comes from. You read, change what you want, and sign.

Into the chart

The signed note goes into the practice software where an interface exists, and is copied into it where not. It becomes a document in the chart like every other, dated and citable, so that the next visit's summary already knows what was said today.

From the note, Mosaic proposes the codes for the visit and the documents for the patient. See Find the right codes and Patient handouts and take-homes.
