The chart, read
Mosaic reads the patient's chart the way you do, from the screen, read-only, and links every statement it makes back to the document it comes from.
Where the chart comes from
Mosaic connects to the practice in two ways. Where your practice software offers an interface, Mosaic uses it. Everywhere else it reads what is on the screen, the way you read it, without writing into the system. Both ways are read-only: the chart stays the practice's chart, and Mosaic keeps its own reading of it.
Documents that arrive on paper or by fax become part of the reading as well: scanned letters and reports are read like typed ones.
The practice software on the left with an open patient file; on the right Mosaic's reading of the same file: 14 documents, each with type, date and title, the newest on top.
What Mosaic reads
- Discharge and referral letters, from your practice and from others.
- Imaging and laboratory reports, with their values and dates.
- Your own notes from earlier visits, and the notes Mosaic wrote with you.
- Medication lists, vaccination records, consent forms, external reports.
Every document gets a type and a date. That is what the answers cite later: a chart citation names the document, its type and its date, and one click opens it.
Click through to the source
A statement in a summary or an answer carries a chip like Lab 08.05.2026 or Ultrasound 12.05.2026. Hover to see the document title and date, click to open the document at the passage. Nothing in the summary is without such a source.

What stays with the practice
Access to the chart belongs to the practice and is granted to the physicians and the team you name. Patients see nothing of the chart unless their physician shares a document with them. Reading and processing happen in the European Union. See Data and security.
