The medication she takes, checked against the drug you are about to start

The medication list and the intake answers read together, each substance checked against the product information of the new drug. The answer says what to do, with the source beside each line, and names the earlier reaction in the chart when there is one.

Does atorvastatin interact with her current medication?

Answered on 16 September 2026

Her current medication: ramipril 5 mg, sertraline 50 mg. Medication 16.09.2026 No clinically relevant interaction with either. Atorvastatin is metabolised by CYP3A4; the interactions that matter are with strong CYP3A4 inhibitors such as clarithromycin, itraconazole and ritonavir, with ciclosporin and gemfibrozil, and with large amounts of grapefruit juice. None is on her list. Eur Heart J

The relevant safety question is the earlier myalgia, not an interaction: start low (10 mg), take it in the evening, document the baseline CK (96 U/l in May) and re-check CK and ALT at eight weeks or earlier if symptoms return. Why was her statin stopped?

Answered on 16 September 2026

Finished thinking

Simvastatin 20 mg was started on 11 January 2024 for an LDL of 158 mg/dl and stopped on 14 March 2024 because of muscle aches in both thighs that began about three weeks after the start. Creatine kinase was 210 U/l (upper limit 170), below four times the upper limit; the symptoms resolved within two weeks of stopping. Visit 14.03.2024 Lab 02.11.2023

Symmetric proximal muscle pain with a normal or mildly raised CK that resolves on withdrawal fits the EAS definition of statin-associated muscle symptoms. The consensus advises a rechallenge with a different statin at a lower dose or alternate-day dosing rather than permanent avoidance; most patients tolerate a second statin. Eur Heart J No rechallenge and no non-statin alternative were documented after March 2024. Medication 16.09.2026
