Years of notes, letters and laboratory read into five lines with their sources

Visit notes, laboratory reports, imaging, external letters and the medication list become one short history, every line citing the document it came from. What happened since the last visit, and where the chart mentions a detail, is one question further.

Where does the chart mention her father's heart attack?

In two places. The intake interview of 20 September 2023 records under family history: “Father: myocardial infarction at 58, stent, alive. Mother: type 2 diabetes, hypertension.” Intake 20.09.2023 The draft of today's ambient note records that she raised the topic herself: “worried since my father's heart attack, he was 58.” Ambient note 16.09.2026

The May check-up documentation does not repeat the family history, and the SCORE2 documented there was calculated without it; SCORE2 does not include family history, which is why the guideline treats a premature family history as a risk modifier on top of the score. A myocardial infarction in a first-degree male relative before 55 counts as premature; at 58 it is borderline and still worth weighing. Check-up 12.05.2026 Eur Heart J + 1
