Criteria met and the step not taken, found across every chart

Mosaic runs the rules the guidelines state against every chart it has read: a criterion the record already meets, and the step that should follow it and is not documented. Each patient comes with what is still to confirm, and nothing leaves the practice.

Which of our patients meet a criterion in their record without the step that follows?

Finished thinking

Four rules, run against the 143 charts this practice has connected. Thirty-one patients to review, each because the record already meets a criterion the guideline names, and the step that follows from it is not documented.

  • Nine patients: two eGFR values between 30 and 59, more than ninety days apart, with no chronic kidney disease in the chart. The criterion for stage 3 is two qualifying values, not one.
  • Six patients: three or more antihypertensives together with repeated hypokalaemia, and no aldosterone-to-renin ratio. Eur Heart J
  • Four patients: LDL above 190 mg/dl on two reports, with no assessment for familial hypercholesterolaemia. Eur Heart J
  • Twelve patients: age, AST, ALT and platelets all present and the FIB-4 never calculated. The score needs no new test. J Hepatol

Every line names the criterion the record meets, not a diagnosis, and no model is guessing: the rules are the ones the guidelines state. The charts were read where they are, and nothing left the practice.

Assembled from 14 documents in the chart. Every line names the documents it rests on. Mosaic states which criterion is met; whether it applies to this patient is the physician's call.
