Your practice · 143 charts connected · nothing leaves the practice
Which of our patients meet a criterion in their record without the step that follows?
Answered on 16 September 2026
Four rules, run against the 143 charts this practice has connected. Thirty-one patients to review, each because the record already meets a criterion the guideline names, and the step that follows from it is not documented.
- Nine patients: two eGFR values between 30 and 59, more than ninety days apart, with no chronic kidney disease in the chart. The criterion for stage 3 is two qualifying values, not one.
- Six patients: three or more antihypertensives together with repeated hypokalaemia, and no aldosterone-to-renin ratio.

- Four patients: LDL above 190 mg/dl on two reports, with no assessment for familial hypercholesterolaemia.
Eur Heart J
- Twelve patients: age, AST, ALT and platelets all present and the FIB-4 never calculated. The score needs no new test.

Every line names the criterion the record meets, not a diagnosis, and no model is guessing: the rules are the ones the guidelines state. The charts were read where they are, and nothing left the practice.
