Mosaic reads a chart from your screen, read-only, and keeps its own reading. A chart is known as far as it has been shown; for the patients of the day the client reads it before you arrive. Where your practice software offers an interface, Mosaic reads the chart in full without the screen. What was never opened is not known, and the answers say so.
